Answer To Question 3: The Large Vaccination Centre do not have access to anyone who may have suffered injury or death, this would be via presentation at GP/ED. Anaphylactic type reactions are reporting via yellow card at the time and all Large Vaccination Centre Clinical Site Managers are aware of the requirement to complete the yellow card at the time. | |
Question Number 4: Number of unique reports to MHRA Yellow Card Reports made by the Trust for Covid-19 vaccinations Please confirm how many reports the Trust has submitted to the MHRA Yellow Card System relating to COVID-19 vaccinations. Please provide a monthly breakdown from November 2020 to December 2021 inclusive | |
